How do you create a questionnaire on a web page?

I own a website through yahoo using the basic website builder. I need my customers to provide me with their information but none of the layouts they have fits my needs. Do I have to use a specific web design software?

You will probably need to create a form where you can grab the information from the customers. This usually requires some programming skills.

Creating a questionnaire is relatively easy, but responding to it requires some knowledge of a server-side programming language like PHP.

Probably the easiest way to do this would be to create a form with Google docs:

You don’t have to know anything about HTML or programming. Just create the form and post a link to the form on your main (yahoo) site. When you want to see the results, go back to Google Docs and the results will be stored as a spreadsheet.

You can also have the results emailed to you when somebody enters the form.
